Welcome to the Community Connectors section

CC_logo_300x210Community Connectors is a new service that can help direct patients to local health services.

If you don’t know where to go to find health advice or information – or you simple want to find out about services in your area that can improve your health and fitness the Community Connectors can help.

What is the Community Connector service?

The project aims to encourage two-way communication between Heart of Birmingham tPCT and its patients to improve the flow of information, particularly within communities that the PCT has been unable to reach through traditional communication methods.

It provides information to the public through community, peer and primary care routes which enables people to make better and more appropriate use of health services.

Who are Community Connectors?

Community Connectors are volunteers who help give out health messages to people in the community. They are people who work the heart of communities and speak with residents on a daily basis. Through their existing jobs or positions in a community, they are ideally placed to advise people about health services are available.

Typically, a Community Connector will be a front line primary care staff member - a receptionist, pharmacist, community worker or staff and schools.

Community Connectors sometimes wear a t-shirt or a badge with the logo on so that people can recognise them.

What can they help with?

From weight-loss services to help finding a nearby doctor through the NHS Choices website, there are a range of services that Community Connectors have information on. For people that are confused about which health service to access, have difficulty finding services because of language barriers or simply need basic advice about health – Community Connectors provide a face-to-face information service that can help.

Where can I find them?

The Community Connectors scheme began at the start of 2010 and currently covers all areas of the Hear for Birmingham Teaching PCT area.
